What To Expect

Life after a brain injury can be challenging for both the individual and their family, often requiring specialist round-the-clock support. At Community Care Experts, we provide specialised brain injury care with years of experience in supporting both traumatic and non-traumatic cases at home and in the community.

Understanding that each brain injury is unique, we collaborate with service users, families, and medical professionals to create personalised care plans tailored to individual rehabilitation goals. Our service integrates neuro-rehabilitation activities to help clients achieve their day-to-day and long-term objectives.

 

We review care plans and risk assessments every three months or as needed, ensuring any changes are promptly communicated and implemented, helping our clients reach their full potential.

Physical and cognitive rehabilitation

Case Managers

Case Managers will be invited to tailor bespoke and client-centred training and supervision for their clients' care team, and we will facilitate it.

All our care packages have a designated team leader.

Clients care plans, risk assessments, daily notes etc., can be accessed digitally via our care control system with no extra cost.

We will invite Case Managers to their clients' team meetings every three months, as and when needed. We will organise and facilitate the meetings.

We will work with case managers to design, implement, monitor and evaluate clients' rehabilitation goals. 

We facilitate and organise the MDT meetings. 

Case managers can access our policy centre, including COVID-19 Hub (QCS)

We have a comprehensive recruitment process including DBS and pre-employment check
