After the diagnosis of long-term neurological conditions such as Huntington's Disease, Multiple Sclerosis (MS), Motor Neurone Disease, Myalgic Encephalomyelitis (ME), Parkinson's Disease and other complex Neurological Conditions, life can become very challenging and daunting. However, you and your loved ones are not alone. Community Care Experts work with you and your loved one and provide full support to you to reach your full potential. Our services are available as minimum as 1 hour depends on the service users location.

  • Living within a 3-mile radius from Surbiton: Minimum 1 hour

  • Living between 3–5-mile radius from Surbiton: Minimum 2 hours

  • Living between 5–10-mile radius from Surbiton: Minimum 6-8 hours

  • Living from 8-mile radius from Surbiton: subject to assessment, minimum support from 6-8 hours, Live-in, awake and sleep-in night

At Community Care Expert, a dedicated member of our team is available to you 24/7 to support you regardless of the level of your need. Our neurological conditions service is flexible and personalised. We always remember that each person has individual needs. Our care packages are based on service users' individual support needs. Care plans and risk assessments are also based on service users' preferred lifestyle, background and wishes. We will review care plans and risk assessments at least every three months and when needed. Furthermore, changes to service users care needs will be implemented and communicated to their care team immediately.

Our complex care packages have a dedicated team leader who will lead regular and backup support workers. The package team leader will maximise the communication amongst the multidisciplinary team and brings continuity and consistency to the service provided. Our skilled managers continually guide and support the team leaders.​

​

All our support workers receive regular visits and supervision from a senior member of Community Care Experts. We will evaluate support workers skills and knowledge regularly to ensure they maintain the outstanding knowledge and skill needed. In addition to the regular supervision, team members will receive clinical supervision at least every six months from our clinical supervisor, an experienced registered nurse with an active NMC pin. Safeguarding is at the heart of everything we do, and our recruitment is subject to successful enhanced DBS and pre-employment checks.
